Service Planning & Review
Each patient seen has very unique and individualized needs which are
addressed through an Individualized Service Plan (ISP). The ISP is
a written agreement between Shaker Clinic and the patient that describes
in detail all services to be provided. The ISP allows:
• A collaborative process to occur between the
patient and provider through involvement of monitoring and evaluating
goals, objectives, and interventions.
• The formulation of a written agreement which shows progress of issues identified
• Assurance of continual communication between the care provider and the patient
• Promotion of personal responsibility
