Intake Process

At Shaker Clinic, we do all that we can to make the intake process as smooth as possible. An initial needs assessment can be done over the phone or in person during our normal hours of operation, between 9:00 am and 5:30 pm. During this assessment, we will inquire about the following, in addition to other relevant information depending on the needs of the potential patient:

  • Primary identifiable concern(s)
  • Psychiatric history
  • Medical history
  • Information pertaining to primary support
  • Information on other services that are being received, if any

If, for any reason, Shaker Clinic is not the service provider that meets your needs, we will provide you with beneficial referrals to another provider, so that appropriate care can be sought and received. However, if needs can be met through our services, the next step will be a meeting with our Clinical Director. This meeting will take place within 48 hours of the initial needs assessment so that services can begin as soon as possible. During this meeting, the Clinical Director will conduct the following:

  • Psychiatric Diagnostic Evaluation

Once needs have been assessed and a diagnosis has been determined, we will begin formulating you Individualized Service Plan (ISP) will be drafted and care at Shaker can commence.
