Each patient seen has very unique and individualized needs which are addressed through an Individualized Service Plan (ISP). The ISP is a written agreement between Shaker Clinic and the patient that describes in detail all services to be provided. The ISP allows:
- A collaborative process to occur between the patient and provider through involvement of monitoring and evaluating goals, objectives, and interventions.
- The formulation of a written agreement which shows progress of issues identified
- Assurance of continual communication between the care provider and the patient
- Promotion of personal responsibility
Shaker Clinic has a standard service agreement in effect with the following community agencies:
- Mental Health Services, Inc.
- NetCare Access, Inc.
The relationships between the above community mental health agency is developed in accordance with terms involving liaison, meeting needs of potential clients, joint clients, transfers, continuity of care and negotiations.Mental Health Services, Inc. and NetCare Access, Inc. provide hotline, referral and information, crisis assessment, crisis intervention, and medication/somatic services to adults and geriatrics.
The above agency will collaborate in the provision of 24-hour-per-day crisis intervention and psychiatric services for clients of Cuyahoga County, Franklin County, and surrounding areas, who are experiencing psychiatric crises.
As a patient at our facility, your privacy is paramount. We are required by both federal and state law to maintain the privacy of your protected health information.
We will not release any confidential health information about you or your treatment without your explicit consent.
